Guidelines on Clinicals
GUIDELINESĀ ONĀ CLINICALS:
- Students intending to have Clinicals should communicate their intent to the assigned Clinical Instructors.
- The clinical instructor orients the student on the process.
- Student secures a Clinicals Request Form from the FCCC Office;
- Student fills-out the form completely and accurately and submit to the FCCC Coordinator;
- FCCC Coordinator checks, verifies, and approves the accomplished Clinicals Request Form;
- The approved Clinicals Request Form will be forwarded to the UV Coordinator and assigned Clinical Instructor ;
- UV Coordinators re-checks the approved form for billing purposes while the instructors conducts a preliminary interview with the student regarding the request for verification purposes;
- Billing will be sent to FCCC for collection;
- Once payments are made, FCCC Coordinator informs UV Coordinator and assigned Clinical Instructor for finalization of schedule and proper arrangements.
- Once finalized, assigned Clinical Instructor will arrange for the Dormitory and Pick-up from the airport.
Important Notations:
- All Clinicals request should be filed at least one month before the desired period of Clinicals
- No student will be allowed to go on Clinicals not unless they made the necessary payment for the Clinicals.
- Any changes after the finalization of the request should be communicated immediately to the FCCC Coordinator or UV Coordinator or the Assigned Clinical Instructor.
